When I was in in my first year in Medical school, I overheard that surgeons usually gist while performing a surgery, I wasn't comfortable with it, i thought opening up a human requires seriousness and not jesting. Growing up, I realised the gist is a coping mechanism, a way to survive through the operation. Doctors are humans too. Doctors also have to be fit in order to stand for long hours.
